At Creekside Vision, we are committed to providing your entire family with a lifetime of professional eye care! We strive to make your experience at Creekside Vision as convenient and pleasant as possible. This area of our website provides the information about what to expect at your appointment and general patient information. In addition, the forms required at the time of your visit are available for download below so you have the convenience of completing them in the comfort of your home prior to your first appointment.

What To Expect At Your First Visit
For all new patients, we ask that you please arrive 15 minutes early to complete any necessary paperwork. And don't worry, we understand visiting an eye doctor can be intimidating, especially if you haven’t been in a while. To make the most of your first visit, please remember to bring a list of the current concerns you need addressed, have your medical history information available, bring a copy of any current prescriptions and over-the-counter medication that you are currently taking, and bring your list of questions that you would like to cover. For eye exams, please bring your current glasses, sunglasses, and contact lenses if applicable. With this little bit of preparation, your first eye exam and evaluation will be simple and straightforward!

*For eye exams, please keep in mind that your first visit can be anywhere from 1-1.5 hours as your eyes require dilation for a comprehensive eye exam and retinal evaluation, along with the personalized service we provide to get to know you and any concerns you may have with your sight. Please be aware that eye dilation does causes sensitivity to light. We recommend you bring a pair of sunglasses with you to your eye appointment. However, if you forget a pair of sunglasses, our Creekside staff will be glad to provide you with a complimentary disposable pair of sunglasses to aid in your comfort on your drive home.

Insurance and Payment Options
We are proud to participate in most insurance programs. If you have insurance through a company that we have contracted with, we will bill your insurance company for the visit. We will require a copy of your insurance card and a driver's license. All co-payments, co-insurances and deductibles are due at the time of visit, no exceptions. You will be responsible for any services denied by your insurance carrier as not medically necessary and/or not covered. We accept Visa, Mastercard, and Discover.

Use it, don't lose it! Don't forget to utilize your Flexible Spending Plan, Health Savings Account (HSA), Medical Savings Account (MSA), and your Cafeteria Plan for your eye care needs!

Care Credit
If you have little or no vision coverage, and you are interested in financing a portion or all of a procedure, we participate with CareCredit. CareCredit offers a variety of plans and quick and easy online credit approval in minutes. Contact our Surgery Scheduler to find out more about financing your procedure at (269) 945-3888.
New Patient Paperwork
If you are a new patient, please download the following new patient paperwork packet. We ask that you please complete this packet and bring it to your first appointment, at least 15 minutes before your scheduled time. This will help to save us time in registering you as a new patient with our Creekside Vision family!
