At Creekside Vision & Hearing we are committed to providing your entire family with a lifetime of professional eye and ear health care! We strive to make your experience at Creekside Vision & Hearing as convenient and pleasant as possible. This area of our website provides the information about what to expect at your appointment and general patient information. In addition, the forms required at the time of your visit are available online to provide the convenience of completing them in the comfort of your home prior to your appointment.

*A parent or legal guardian MUST accompany any minor under the age of eighteen to all appointments. If a parent or legal guardian can not accompany the minor a "consent for treatment" form must be completed at our office prior to the minor's appointment. We will then keep your "consent for treatment" on file should the minor need to return at a future date for additional appointments.

Request an Rx Refill

Contact our office directly at (269) 945-3888 if your prescription has expired or if no more refills are available and further assistance is needed.

Care Credit

If you are interested in financing a portion or all of a procedure, we participate with CareCredit. CareCredit offers a variety of plans and quick and easy online credit approval in minutes. Contact our Surgery Scheduler to find out more about financing your procedure!


Insurance Coverage

If you have insurance through a company that we have contracted with, we will bill your insurance company for the visit. We will require a copy of your insurance card and a driver's license. All co-payments, co-insurances and deductibles are due at the time of visit, no exceptions. You will be responsible for any services denied by your insurance carrier as not medically necessary and/or not covered. All major credit cards are accepted.
